Complimentary Consultation

Consultation with Dr. Miyamoto and Dr. Tamashiro

All of our potential new patients receive a complimentary consultation. We understand that choosing to complete orthodontic treatment can be a tough decision to make. At Cinema Orthodontics, we strive to inform you of every aspect of your diagnosis and treatment so that you feel comfortable making that decision.

The American Association of Orthodontists recommends that all children should get an orthodontic evaluation no later than age 7. Though treatment is not always indicated at that time, an early evaluation by Dr. Miyamoto and/or Dr. Tamashiro can help avoid any future complications

What to Bring to the Initial Consultation

• New Patient Form
• Insurance card, if applicable. We accept most PPO’s and HMO’s.
• Referral form from dentist or insurance, if applicable

What to Expect at the Consultation

• Tour of the office
• Complimentary panoramic x-ray, cephalometric x-ray, photos, and a digital intra-oral scan
• Evaluation from Dr. Miyamoto and/or Dr. Tamashiro

Second Consultation (Treatment Plan)

If You are Planning to Transfer from a Previous Orthodontist

Please provide us with as much information as possible from the other office, such as:

• X-rays, pictures, or impressions
• AAO Release or Transfer Form
• Brief explanation by the previous orthodontist of what was already completed
